This program’s innovative design is rooted in the belief that everyone, regardless of income, immigration status, home language, or zip code, should have access to high-quality, equitable, and inclusive healthcare for themselves and their children. While the program’s primary goal is to bridge the public health system and community resources for patient families served by the hospital, the community promoters leading this program provide holistic support to the whole family while addressing systemic barriers.
Data gathered since the launch of the program in mid-2020 shows that CHLA had a patient connection rate of 26%. Since then, through the work of the community promoters, CHLA has achieved an 85% connection rate. This success enables patient families, particularly monolingual Spanish-speaking, to access needed food, housing, childcare, legal, health, and financial assistance.
The CPTS team developed a manual and power-shifting tool that offers a scalable framework for public health, education, housing, early care and education, and other systems to adopt that will promote social justice and improve outcomes for marginalized communities. Modeled after PLN-CTD’s Best Start Community Partnership and community promoter model, this program consists of the following components:
The power-shifting tool is intended to support institutions who have existing promoter programs and those who wish to implement one. The tool will help institutional champions to 1) assess their institution's workplace policies and practices towards inclusive and equitable integration of non-English monolingual speakers; and 2) understand and advocate for the changes needed to improve their policies and practices to strengthen the integration of this role.
